The following is a list of forms available for download as PDF. You may use the PDF viewer of your choice. However, Adobe Acrobat Reader 7.0 or later is required to take advantage of the online form capabilities. You may fill the forms out and print a copy for your records. Due to their sensative nature Kids Kondo cannot accept digital signatures. Therefore, it is advised that the forms be faxed or hand delivered to Kids Kondo after they have been completed.
Enrollment Application
This form is for registration and it is your financial agreement for enrollment. Use it to be placed on the waiting list. Fill out once and update information as needed.
Child Enrollment & Medical Care Form
This should be filled out prior to start date (after the child is born) and updated as needed.
Parental Agreement
Signed agreement between you and the center that we each have certain things to abide by.

Infant Feeding Plan
This form is for children 12 months and younger. It must be updated every six weeks.
Medication Authorization
For us to dispense any medication this form is required and must be filled out completely.
